Admission to the Clinique Chirurgicale de l'Espérance
Admission:
In all cases, a physician must request hospitalization. Except in emergencies, admissions take place every day between 3:00 pm and 5:00 pm. Minors are only admitted after receiving the permission of either parent, the child's legal guardian, or a legal authority, except in emergencies. The admission of an adult under protective supervision is subject to the consent of the patient or the patient's legal representative (guardian, trustee, etc.)
Please respect the admission time communicated to you by the admissions department and doctor's office.
Registration:
At the time of admission, a patient, or a patient's family, must indicate his or her identity and address, and health insurance (national health insurance card) or pension fund (membership of health care card) coverage. To speed up the admissions formalities, pre-admission forms are available in the offices of physicians practicing at the institute and in the admissions office. This document must be completed and submitted as soon as possible (regardless of the planned admission date) and include a photocopy of the patient's health insurance card and, if relevant, supplementary health insurance card.

Documents to Present on the Day of Admission:
► National identity card
► Family record book for minors
► Passport or residency card (for non-citizens)
► Health insurance or entitlement card
► Supplementary insurance card (if relevant)
Health care coverage:
► Prepayment is not necessary for patients insured by the national health insurance plan (Caisse Primaire d'Assurance Maladie) of Alpes-Maritimes and Var, and certain other health insurance plans (list available in the admissions office). Just present your health insurance card indicating the date of validity of your coverage (or "carte vitale" with its certification document).
► If the patient is not the insured, the patient must confirm that the required formalities have been fulfilled for the insurance plan of the claimant (spouse, domestic partner, children, etc.).
► If the patient receives benefits according to article 115 of the military pension code, present the form for free medical treatment and the medical testing form.
► If hospitalization involves a workplace accident, present the report issued by the employer.
► Prepayment is not necessary if, prior to admission, the health insurance or supplementary insurance plan has approved the coverage of additional charges for private room, flat per day rate, copayment, etc.
Personal Objects:
Please bring the following personal objects with you:
► Toiletry kit (220 V)
► Nightshirt or pajama
► Robe
► Disposable tissues
► Slippers
► Change of underwear
► Napkins and a hand towel
► Test results, blood group card, X-rays, and medical records, to be given to the nurse in the department you are staying in (please do not forget to recover these documents when you are discharged). Also bring your health card (carnet de santé).
► Holder or case for dentures, hearing aids, or glasses, if necessary.
Bring as few objects, valuables, and cash as possible.
The clinic management only guarantees items stored in the clinic's safe.
Act of July 6, 1992 - Order of March 27, 1993. Sums of cash and valuables... as well as other property the nature of which justifies safekeeping (keys, identity papers, etc.) can be stored in the safe of the facility. In no case is it mandatory to store cash, but the facility is only liable for stored property, except in the case of persons unable to carry out the storage procedure at the time of admission. After a period of one year, abandoned items (items not retrieved or stored) will be turned over to the Caisse des Dépôts et Consignations (French Deposit and Consignment Office) for their value, and to the state property service to be put on sale for other goods.
The institution assumes no liability for items entrusted to staff members or stored in personal safes without the knowledge of the institution.
Charges Payable on Admission:
For patients with national health care insurance and those with supplementary insurance, you will be asked to make a payment:
► If you do not have the annual proof of valid coverage, or your health insurance card has expired.
► If your coverage is not 100%. In this case, the requested payment will not exceed the legal copayment for the length of hospitalization, and for no more than one month of hospitalization.
► For patients insured outside this département, and affiliated with health insurance plans restricting the amount of coverage to that closest to their place of residence (except in emergencies), the difference must be paid by the patient if the covered amount is less than that of our clinic.
Patients billable at a flat per day rate as stipulated by Act no. 83.25 of January 19, 1983, will be asked for a payment based on the amount indicated by statutes currently in force and the estimated length of hospitalization.
► Completely uninsured patients, will be asked to pay the entirety of their estimated hospitalization fees.
In all cases,a payment will be requested from patients desiring a private room, telephone, or television.
► In addition to supplementary charges, all hospitalization fees, as well as doctor's fees, must be paid at the time of discharge if you have not provided the documentation requested on the day of admission.

Your stay
During your hospital stay, our staff of dedicated
professionals is at your disposal. If you have any
questions or needs, please see the charge nurse on
your floor or the receptionist located in the lobby.
The receptionist is available weekdays from 7:00
am to 6:00 pm, except Saturdays, from 8:00
am to 4:00 pm and Sundays, from 10:00
am to 6:00 pm.
The Clinic Staff:
Each staff member has specific and well-defined responsibilities. Their goal is to provide high quality care and make your stay as comfortable as possible.
They are:
► Surgeons and medical specialists
► Charge nurses, responsible for the management and operation of the departments
► Public health nurses
► Nurses responsible for administering your nursing care. They see to your comfort and well-being.
► Health care assistants who help provide ongoing basic care and services (hygiene and housekeeping) and assist nurses
► Dieticians
► Physical therapists
► Psychologists, speech therapists, and podiatrists who are available to patients through medical referral (additional fees apply).
► Medical informatics department (DIM), which processes documentation relating to your hospitalization and your medical record
► Administrative staff

Your Discharge to the Clinique Chirurgicale de L'Espérance

Your stay is ending
Discharges take place every day between 9:00 am and 11:00 am, according to your physician's orders. The charge nurse on your floor will give you your test results and X-rays. Remember to pick up your new and previous X-rays and echographic images; the clinic does not store these documents.
Before your departure, it is mandatory to go to the discharge office to finalize your administrative record and settle the fees for your medical care.
You would like to end your stay early
If you request discharge against medical advice, you must sign a waiver relieving the physician and clinic of all liability.
If required by your state of health, and only under a physician's order, you can be transported by taxi or private ambulance. In this case, ask the physician providing your hospital care for a medical certificate attesting to your need for this kind of transportation.
